The following forms are to be used for Ocean shipments of Dangerous Goods (Hazardous) and can be filled out online and emailed back to International Forwarders, Inc. To do this, fill out the form and save the completed form to your local computer. Then open your email program and attach the completed document to an email and send it to hazdec@ifichs.com. If you prefer, the completed form can also be printed & faxed back to International Forwarders, Inc. at (843)-769-7042.

IMO Dangerous Goods Declaration with Container/Vehicle Packing Declaration

This form or the next two forms are to be used for full container shipments of Hazardous Goods. If this form is used then the other two don.t need to be filled out. If the other two forms are used then this form does not need to be filled out.

IMO Dangerous Goods Declaration without Container/Vehicle Packing Declaration

This form is to be used when shipping LTL, "Less than Container Load", shipments or when the person filling out the form is not the same person packing the container for full container shipments

If you are shipping a full container of Hazardous Goods and using this form then the Container/Vehicle Packing Declaration, "CPC", needs to be filled out by the person packing the container. Both forms will need to be sent back to International Forwarders, Inc. via email or fax if you go this route.

Container/Vehicle Packing Declaration, "CPC"

This form needs to be filled out when the above, "IMO Dangerous Goods Declaration without Container/Vehicle Packing Declaration", form is used and you are shipping a full container of Hazardous Goods. This form does not need to be filled out for LTL, "Less than Container Load", shipments.
