IMO Dangerous Goods Declaration

The following forms are to be used for Ocean shipments of Dangerous Goods (Hazardous) and can be filled out online and emailed back to International Forwarders, Inc.  To do this, complete the form and save it to your local computer.  Once saved, open your email program and attach the completed document to an email.  For email instructions please contact your IFI representative.  If you prefer, the completed form can also be printed and faxed to International Forwarders, Inc. at (843) 769-7042.

This form or the next two forms are to be used for full container shipments of Hazardous Goods.  If this form is used, the other two do not need to be completed.  If the other two forms are used, this form does not need to be completed.

  Example of a completed form

This form is to be used when shipping 'Less Than Container Load' (LTL) shipments, or when the person completing the form is not the same person packing the container for full container shipments.

If you are shipping a full container of Hazardous Goods and using this form, the Container/Vehicle Packing Declaration (CPC) needs to be completed by the person packing the container.  Should you go this route, both forms will need to be sent back to International Forwarders, Inc. either via email or fax.

This form needs to be completed when the above 'IMO Dangerous Goods Declaration without Container/Vehicle Packing Declaration' form is used, and you are shipping a full container of Hazardous Goods.  This form does not need to be filled out for LTL shipments.
